Job summary

Valley Manor Inc. in Ontario is seeking a registered Dietitian to oversee resident nutrition, hydration and medical nutrition therapy in a long-term care setting.

The role requires adherence to professional standards and collaboration with the inter-disciplinary team to protect residents’ well-being. The successful candidate will bring current licensure, strong communication skills, and the ability to manage multiple priorities with compassion and efficiency.

Qualifications

  • Registered Dietitian in Ontario and in good standing.
  • Liability insurance, TB test and vulnerable sector police check are required.
  • Experience in Long Term Care is an asset.
  • Ability to work in a team and independently.
  • Strong verbal and written communication skills.
  • Excellent organizational and time management skills.
  • Knowledge of EMR systems; Point Click Care is an asset.

Responsibilities

  • Identify and monitor nutrition and hydration risk for Residents.
  • Complete RAI-MDS assessments and assess nutrition status on admission and with changes.
  • Prioritize and respond to dietary referrals promptly.
  • Develop and modify nutrition care plans with the inter-disciplinary team.
  • Conduct swallowing assessments as needed.
  • Provide nutrition counselling to Residents and families.
  • Collaborate with Food and Nutrition Manager to ensure a pleasurable dining experience.
  • Participate in Care Conferences and meetings as needed.
  • Review menus and develop individualized menus as required.
  • Educate dietary and nursing staff to support home policies and procedures.

Education & Experience
  • Member in good standing of the College of Dietitians of Ontario
  • Must have liability insurance, TB testing and vulnerable sector police check
  • Membership in Dietitians of Canada – Gerontology Network an asset
  • An active member of CSNM is considered an asset
  • Experience in Long Term Care is an asset
  • Ability to work in a team and independently
  • Good verbal and written communication skills
  • Excellent organizational and time management skills
  • Demonstrates an ability to problem solve
  • Computer Literacy - knowledge of computerized electronic medical records (EMR), Point Click Care programs an asset
Responsibilities
  • Identify and monitor level of nutrition and hydration risk for all Residents.
  • Complete scheduled RAI-MDS assessments; assess nutritional status for all Residents upon admission, annually and when significant changes in status occur.
  • Prioritize and respond to dietary referrals in a timely manner.
  • Develop, evaluate and modify nutritional care plans for all Residents; liaise with Residents, family members and/or substitute decision makers and the interdisciplinary team when implementing dietary interventions.
  • Conduct swallowing assessments, as needed.
  • Provide nutrition counselling and education to Residents and family members and/or substitute decision makers, as needed, in line with supporting Residents’ nutrition care.
  • Collaborate with the Food and Nutrition Manager to implement a pleasurable dining experience for Residents.
  • Participate in Care Conferences and committee meetings as needed.
  • Review and approve seasonal menus; develop individualized menus for Residents, as needed.
  • Provide education and training to dietary and nursing staff to support home-specific policies and procedures, as needed.
  • Participate in home-specific continuous quality improvement initiatives, as needed.
