MOSAIC is partnered with the Burnaby Division of Family Practice, the Burnaby Primary Care Network, and Fraser Health to operate the MOSAIC Community Clinic in the Burnaby Edmonds area. The clinic will support four priority populations : newcomers, new mothers and babies, homeless or under-housed individuals, as well as individuals with complex mental health. The clinic is multi-disciplinary with family doctors, nurse practitioners, administrative staff, and Allied Health practitioners.

POSITION SUMMARY :
Reporting to the Clinic Director, the Registered Clinical Counsellor (RCC) is responsible for providing consultation, assessment, and case management services. Utilizing a team-based care model the Clinical Counsellor works closely with Family Physicians, Nurse Practitioners, Registered Nurses, and other allied health professionals to provide patient care to individuals and families affected by mental health disorders and / or substance use. The position requires collaboration with an interdisciplinary shared care team to develop shared care plans and provide comprehensive case consultation and follow-up support. In accordance with the mission, vision, values, and strategic directions of MOSAIC, client / patient safety is a priority, and a responsibility shared by everyone within the organization, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

The successful candidate must be able to commute to our Edmonds Clinic location.
COMPENSATION : $44.00 -$55.00 per hour
BENEFITS :
A competitive benefits package is provided with contracts of at least 1 year in length and which offer a minimum of 17.5 hours of work per week.
