MOSAIC is partnered with the Burnaby Division of Family Practice, the Burnaby Primary Care Network, and Fraser Health to operate the MOSAIC Community Clinic in the Burnaby Edmonds area. The clinic will support four priority populations : newcomers, new mothers and babies, homeless or under-housed individuals, as well as individuals with complex mental health. The clinic is multi-disciplinary with family doctors, nurse practitioners, administrative staff, and Allied Health practitioners.

POSITION SUMMARY :

Reporting to the Clinic Director, the Registered Clinical Counsellor (RCC) is responsible for providing consultation, assessment, and case management services. Utilizing a team-based care model the Clinical Counsellor works closely with Family Physicians, Nurse Practitioners, Registered Nurses, and other allied health professionals to provide patient care to individuals and families affected by mental health disorders and / or substance use. The position requires collaboration with an interdisciplinary shared care team to develop shared care plans and provide comprehensive case consultation and follow-up support. In accordance with the mission, vision, values, and strategic directions of MOSAIC, client / patient safety is a priority, and a responsibility shared by everyone within the organization, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

KEY DUTIES AND RESPONSIBILITIES :

• Maintain and handle confidential information appropriately, maintaining a working knowledge of best practices with applicable Acts.
• Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
• Conducts assessments, identifying client needs and available services and contributes to the shared care plan with the patient's input.
• Refers patients to appropriate community services and / or rehabilitative programs; through methods such as researching available services, liaising with the service and program providers and providing advice and consultation to colleagues in order to ensure patients receive appropriate and timely service delivery and continuity of care.
• Carries out treatment plans and interventions, provision of support and crisis intervention, including facilitation of hospitalization where necessary. Follows up, monitors, and evaluates treatment outcomes.
• Provides a culturally safe and trauma-informed perspective in consultation with team members to enable patients to access the services they need.
• Communicates and consults with the care team regarding client care and progress.
• Provides referral to appropriate agencies and collaborative information sharing for continuance of care.
• May deliver mental health and mental well-being education sessions to clients and their families in groups or one-on-one.
• Identifies gaps and problems with service and recommends solutions to assist in the development and maintenance of a coordinated mental health care system within the community.
• Uses evidence-based resources, reference guides, in-services and workshops, to keep knowledge, procedures, and current certification.
• Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.
• Provides education and information sessions regarding mental health and mental wellness to the care team, and MOSAIC staff in general.
• Maintains written reports and statistics and prepares written reports and clinical documentation according to established policies and procedures.

QUALIFICATIONS :

• Master's Degree in Counselling Psychology, Social Work, or related field; and current registration in BC Association of Clinical Counsellors
• Two (2) years of recent related experience as a clinical counsellor and demonstrated experience working within an interdisciplinary team environment, or an equivalent combination of education, training and experience;
• Valid and Satisfactory Criminal Record Check Required - Vulnerable Sector Check
• Comprehensive clinical counselling and therapy skills including in crisis counselling
• Demonstrated ability to make effective referrals, reflecting in-depth knowledge of mental health resources and strong collaborative skills in working with mental health and social agency personnel
• Demonstrated ability to work independently and collaboratively as a member of a multi-disciplinary team and to understand the frameworks of other disciplines.
• Demonstrated ability to communicate effectively both verbally and in writing with clients, families, staff members, and community agency personnel.
• Demonstrated ability to educate and provide consultation to clients, families and healthcare professionals regarding psychosis and group and family therapy
• Ability to utilize and interpret client assessment tools
• Demonstrated ability to develop, coordinate and deliver psychoeducation and therapy groups, in-services and presentations
• Ability to work collaboratively and co-operatively with other care providers to facilitate and enhance interdisciplinary, inter-departmental, and interagency communication
• Ability to prepare comprehensive written assessments.
• Ability to communicate effectively in English both verbally and in writing; other languages an asset
• Ability to deal effectively with others including the ability to work in a team and client-focused environment.
• Standard office equipment; knowledge and experience with basic computer programs (EMR, Word, Microsoft Outlook).

HOURS :

full-time position (37.5 hours per week)

The successful candidate must be able to commute to our Edmonds Clinic location.

COMPENSATION : $44.00 -$55.00 per hour

BENEFITS :

A competitive benefits package is provided with contracts of at least 1 year in length and which offer a minimum of 17.5 hours of work per week.
