Job summary

Alliance for Healthier Communities is seeking an OHT Clinical Coach – Palliative Care Project to lead education, training and resource coordination across long-term care, community and hospital sectors.

The role emphasizes equity-based, person-centered palliative care, mentorship of staff, quality improvement, and collaboration with Ontario Health directives; travel within the catchment is required.

Qualifications

• Minimum 5 years of palliative care practice.
• Experience supervising clinical staff across care settings.
• Experience across diverse health-care settings and disciplines.
• Knowledge of Ontario health directives and legislation.

Responsibilities

• Education, training and resource coordination across sectors.
• Provide palliative care mentoring for service providers.
• Support change management and quality improvement activities.
• Collaborate with regional partners to address barriers and improve access.
• Participate in data collection and reporting as required.
