Job summary

Autism Services in Saskatoon is seeking a Pay Band Social Worker - BSW to join the Connecting to Care team on a temporary full-time basis.

The Case Manager provides social case management support as a primary care provider to clients who face engagement barriers related to substance use and mental health. A BSW and at least 1 year in health settings are required, with a valid Class 5 license and the ability to work days/evenings/weekends.

Qualifications

• Bachelor of Social Work (BSW) is required.
• Minimum 1 year experience in a health-related field.
• Valid Class 5 driver's license.

Responsibilities

• The Case Manager provides social case management support as a primary care provider to the client.
• The population for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse/who are impacted by substance use disorder and Mental Health issues.
• Utilizing an outreach approach, the case manager provides client services including psychosocial assessments on intake, addiction assessments, client goal setting/skill building, and coordination/support of health/social services.
