Job summary

Sienna Senior Living's Muskoka Shores Community in Gravenhurst is seeking an experienced RPN RAI Coordinator to lead the planning, implementation and evaluation of the RAI program, ensuring compliance with MOHLTC requirements and internal policies.

Reporting to the Director of Care, you will schedule resident assessments, verify RAI accuracy, generate reports for leadership, supervise data submissions, and drive education for the care team while upholding residents’ dignity and quality of life.

Qualifications

  • RPN certification and active license in good standing with the Ontario College of Nurses.
  • Relias-MLTC RAI certification completion as per MOHLTC requirements.
  • RAI experience or education is an asset.

Responsibilities

  • Plan, implement, coordinate and evaluate the RAI program.
  • Establish, maintain and communicate resident assessment schedules.
  • Ensure RAI accuracy and updates to coding, care plans and MOHLTC requirements.
  • Review and analyze residents' RAI assessments for accuracy and needs.
  • Monitor and analyze RAI outputs and reports to support decisions.
  • Produce RAI reports for leadership use in clinical and operational decisions.
  • Manage computerized data submission and ensure data integrity with leadership.

What you’ll be doing:

The RPN RAI Coordinator is responsible for the planning, implementation, coordination, and evaluation of the RAI Program, ensuring compliance with quality management standards, legislative requirements, and organizational policies and procedures.

You will be a welcomed member team and impact in the following ways:

  • Establish, maintain, and communicate current resident assessment schedule to inter-professional team.
  • Ensure RAI accuracy and compliance with coding, capping and care plans are updated.
  • Reviewandanalyzeeachresident’sRAIassessmentstoensureaccuracyandmeet theresidentsneeds.
  • Monitor and analyzes RAI outputs and reports.
  • Produce RAI reports for leadership use to assist in clinical and operational decision-making
  • Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC). Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization.
  • Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees.
  • Lead the RAI education process for all participating team members
Who You Are:
  • ARegisteredPracticalNurseingoodstandingtheOntario CollegeofNurses
  • Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC
  • RAI experience/education is an asset
  • Good problem solving and analytical skills
