Job summary

VHA Home HealthCare in Toronto, Ontario, is seeking a Quality Improvement Lead to drive organization-wide quality initiatives and accreditation readiness. You will coach leaders, design measurement plans, and apply Lean/Six Sigma methods to improve outcomes in home health settings.

You will partner with cross-functional teams, analyze data to identify trends, and sustain gains through coaching and education, contributing to a culture of continuous improvement across the organization.

Qualifications

• Master's degree in quality improvement, health sciences, healthcare or business administration, public health, health informatics or a related discipline.
• Minimum five years of progressive experience in healthcare quality improvement, performance improvement, accreditation, data analytics or a related field, including experience leading or facilitating cross-functional improvement initiatives.
• Demonstrated knowledge and practical application of quality improvement methodologies and improvement science.
• Demonstrated ability to use, analyze interpret and communicate quality and performance data to identify opportunities, evaluate outcomes and support decision-making.
• Demonstrated experience supporting healthcare accreditation.
• Lean Six Sigma Green Belt certification.
• Proficiency in using data analysis, visualization and process-mapping tools to support quality improvement including Excel, Visio, Whiteboard and related mapping or statistical analysis tools.
• Experience in change and/or project management.
• Experience providing education, coaching and facilitation.
• Strong written and verbal communication skills, including the ability to translate findings into clear recommendations.
• Ability to work independently and collaboratively, manage competing priorities and build effective relationships within a complex, fast-paced healthcare environment.

• Coordinate and support organizational accreditation readiness through standards monitoring, evidence management and action plan completion.

Reporting to the Director of Quality, the Quality Improvement Lead provides organization-wide expertise in quality improvement, accreditation readiness, and the use of performance measurement and data to drive better outcomes.

The Lead advances initiatives aligned with VHA's strategic priorities and Quality Improvement Plan, while building organizational capability to apply improvement methods, use measurement, sustain meaningful change and maintain accreditation readiness.

The role provides consultation, coaching, facilitation and education to leaders and teams in applying quality improvement methods and evidence-informed approaches, while fostering a culture of learning and continuous improvement.

Pay Range

Base Salary: $70,000- $87,000 (commensurate with experience)
