Quality and Risk Officer – Permanent Full-Time

Position summary:

The Quality and Risk Officer will collaborate with senior management and all organizational teams to provide expertise in quality management and risk mitigation. Reporting to the Director of Quality, Risk, and Privacy, the incumbent will support the planning, coordination, implementation, and evaluation of hospital-wide and department-specific quality, risk and patient safety programs. Additionally, the Quality and Risk Officer will foster a culture of excellence by promoting best practices through consultation, education, and oversight of the organization's Improvement Model.

Responsibilities include:

  • Investigate/analyze patient safety events reported through online incident reporting system, ensuring appropriate recommendations are effectively implemented.
  • Maintain and support online incident reporting system while encouraging a culture of transparency, accountability, and continuous reporting.
  • Develop comprehensive, action-oriented incident reports at the unit/departmental/organizational level.
  • Oversee the quality-of-care review process and ensure the implementation of preventative measures.
  • Lead thorough investigations of patient complaints, particularly those of a clinical nature, and facilitate timely resolution through proactive engagement.
  • Provide education on the clinical complaint management process and associated policies, incident reporting procedures, and the SRLS system.
  • Assist in the development and enhancement of clinical risk categories within the Enterprise Risk Management Framework.
  • Coordinate hospital-wide initiatives aimed at reducing, mitigating, or eliminating identified clinical risks.
  • Oversee all HIROC claims and ensure the organization's insurance policies remain current/compliant.
  • Support the development, submission, and evaluation of annual Quality Improvement Plans (QIP).
  • Monitor the progress of QIP initiatives, collaborating with stakeholders to ensure alignment with organizational objectives.
  • Develop/update policies in alignment with investigation outcomes, incident analyses, patient safety and quality standards.
  • Deliver education and training on quality improvement, risk management, and patient safety initiatives.
  • Generate quarterly patient safety reports, focusing on key indicators such as pressure injuries, medication errors, and patient falls.
  • Identify and report negative trends in patient safety data to leadership, facilitating proactive intervention.
  • Support teams in analyzing and interpreting quality and safety data to identify opportunities for improvement, leveraging key tools and best practices.
  • Contribute to the development of methodologies, structures, tools, and templates that support excellence in quality improvement and patient safety.

The successful candidate will have the following qualifications/experience:

  • Current registration in a regulated Nursing Profession (Good standing with CNO)
  • Certifications such as Certified Professional in Healthcare Quality (CPHQ) or Certified Professional in Healthcare Risk Management (CPHRM) preferred
  • Extensive knowledge of the quality, patient safety, risk management literature and applicable legislation
  • Minimum 3 years in a clinical setting
  • Minimum 1 year in quality, risk, and patient safety within a healthcare setting
  • Experience in the complex continuing care or rehabilitation sector is considered an asset
  • Advanced proficiency with adverse event management systems
  • Proficiency in use of office support software (Word, Excel, PowerPoint) and quality tools software packages i.e. Visio
  • Proven leadership skills with the ability to coach, mentor and motivate an inter-professional team.
