Job description

• Registered Nurse (RN) with a current MA license.
• Minimum of two (2) years of home health experience, including experience in QA of a home care agency and OASIS experience.

Knowledge and Skills

• Working knowledge of Medicare regulations, State regulations, and home health documentation requirements.
• Good organizational, interpersonal, and judgment skills.
• Ability to develop action plans and resolve identified inconsistencies.

Responsibilities

• Review and process OASIS documents (Admissions, Recertifications, Resumptions, Transfers, and Discharges).
• Review nurses' notes and other related documents.
• Monitor documentation trends through chart reviews; maintain high documentation standards.
• Conduct Clinical Record Reviews (CRR) focusing on documentation verification, quality of care, visit utilization, adherence to the plan of care and policies, and inter-disciplinary communication to ensure proper documentation for reimbursement.
• Maintain agency CRR compliance at 80% or greater.
• Communicate effectively with all levels of agency staff.
• Report trends and issues to the Clinical Manager / Administrator, including process changes or staff training needs based on QA data.
• Participate in agency orientation and in-service training programs.
• Prepare QA meeting notifications and compile reports from other agency committees for QA program integration.
• Assist in clinical training programs and development of training materials.
• Participate in the agency's emergency preparedness plan.
