City of Hamilton is seeking a Public Health Nurse (PHN) to join the Infectious Disease Program. The PHN will apply public health nursing principles to promote and protect population health, including immunizations and disease prevention.

The role involves case and contact management, collaboration with community partners, and responsiveness to public health threats, with responsibilities aligned to provincial standards and organizational policies.

Formación

• RN with current registration in the general class, RN (Ontario)
• Baccalaureate Degree in Nursing from a recognized program; public health nursing prep is a plus
• Experience in public health nursing is preferred
• Ability to adhere to Ontario Public Health Standards and professional standards

Responsabilidades

• Maintain immunization competencies and vaccine administration.
• Perform case and contact management for diseases of public health significance.
• Collaborate on population health planning and health equity initiatives.
• Provide nursing care using patient-centered, trauma-informed approaches.
