Job summary

SE Health in Downtown Toronto is hiring a part-time registered practical nurse (RPN) for the Women’s College Clinic. You’ll assess clients, develop and implement care plans, and work with a multidisciplinary team to ensure high-quality care.

The role offers flexible scheduling, paid training, and a competitive compensation package, including a $2,000 sign-on bonus and benefits. Travel may be required, with opportunities for growth within SE Health across Canada.

Qualifications

  • Must be registered with the College of Nurses of Ontario.
  • CPR certificate required.
  • Valid driver’s license and vehicle; travel required.
  • Updated immunization records and clear background checks.
  • Willingness to attain Saint Elizabeth certifications.
  • Availability to work days, evenings and weekends.
  • Physically capable of lifting up to 55 pounds and related tasks.

Responsibilities

  • Perform comprehensive client assessments and develop care plans.
  • Act as primary provider for assigned client caseload and coordinate with care team.
  • Support client through transitions by linking with community services.

Responsibilities:
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Requirements:
  • Current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification an asset
  • Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
  • A current CPR certificate is required
  • A valid driver’s license and vehicle (travel required)
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required Saint Elizabeth certifications.
  • Availability to work days, evenings and every other weekend.
  • This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
