111013 ParaMed Inc. in Belleville is seeking a Registered Practical Nurse to perform scheduled community visits, delivering ethical and safe nursing care with support from RNs for complex patients.

You will assess patient needs, implement and monitor care plans, and maintain accurate records while traveling within the designated area. On-call coverage and bilingual skills are preferred where possible.

Qualifications

• Registered with the CNO as an RPN in good standing.
• Valid driver's license and access to a vehicle.
• Experience in delivering community care is an asset.
• Willingness to travel within a designated area.

Responsibilities

• Participate in scheduled visits in the community to care for patients.

Overview

In this community nursing role, as a Registered Practical Nurse (RPN) you will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What you’ll do

• Provide ethical and safe nursing care
• Use appropriate assessment tools/techniques to assess patients’ individual needs
• Execute and continuously monitor care plans and adjust as required
• Engage patients using a collaborative approach by building rapport and trust
• Maintain and update professional patient records related to patient encounters and plans

Compensation

Part time: $26.42 - $30.50 per hour
