Responsibilities:

  • Complete documentation and maintain accurate records, including program guides, participation tracking, monthly statistics, intake and quarterly documentation, MDS/RAPS, care plan updates, and assessments for residents' recreation and rehabilitation needs.
  • Plan and execute programs based on resident assessments, introducing creative and innovative recreational activities regularly.
  • Organize special events, community programs, and outings.
  • Evaluate program effectiveness and adapt as needed.
  • Assess equipment needs and ensure residents have access to appropriate assistive devices (e.g., wheelchair, walker).
  • Participate in department, unit, and multidisciplinary care meetings.
  • Maintain effective communication and positive relations with residents, families, volunteers, and staff.
  • Keep posters, notices, and calendars current, attractive, and legible for residents.
  • Coordinate media notices about programming with the Programs Manager.
  • Develop and sustain community contacts and resources.
  • Collaborate with community agencies to deliver programming.
  • Understand and contribute to quality indicators used by the Home, including public reporting and improvement efforts.
  • Perform other duties as assigned.

Qualifications:

  • Post-secondary diploma, degree, or certificate in recreation and leisure studies, therapeutic recreation, gerontology, or related fields from an Ontario post-secondary institution, or equivalent from another jurisdiction.
  • Or, in the Home's judgment, possess the skills, knowledge, and experience to perform the duties effectively.
  • Experience working with seniors, familiarity with GPA, PIECES, U FIRST, and statements from Butterfly.
  • Experience in planning, implementing, and evaluating programs.
  • Proficiency in computer skills.
  • Excellent communication and organizational skills.
  • Knowledge of RAI-MDS; AIS certification is preferred, or to be completed within one year of hire.
  • Second language skills (Chinese, Portuguese, Italian, Eastern European languages) are an asset.
  • CPR certification.
  • Ability to demonstrate empathy and turn tasks into positive social interactions.
  • Commitment to legal and ethical standards, promoting residents' rights, and risk management.
  • Current (within six months) Vulnerable Sector Check and Criminal Background Check, or willingness to obtain.
  • Proof of TB testing or chest X-ray as per health regulations.
  • Two supervisory references.
  • COVID-19 vaccination required.

What We Offer:

  • Support for personal and professional growth.

We look forward to connecting with you and Making Every Moment Matter. We value diversity and inclusion; accommodations are available upon request during the recruitment process.
