Job Summary:
The Program Coordinator, Hospice is responsible for supporting the Manager and Supervisor, and leading the day-to-day operations within the framework of VON's philosophy, policies, procedures, standards, and quality management.
Key Responsibilities:
- Implement, maintain, and operate 1:1, group, and virtual care solutions required by VON for service delivery.
- Support inquiries from community members, clients, and volunteers about the Hospice and Bereavement programs.
- Provide training to new and existing Hospice Services volunteers.
- Coach, mentor, and debrief with volunteers regularly.
- Perform client assessments to determine program suitability and eligibility.
- Maintain training materials for program administration.
- Navigate resources and systems to make appropriate external referrals.
- Build and maintain community relationships related to hospice and bereavement care.
- Ensure compliance with VON policies and procedures.
- Provide cost-effective client services and programs.
- Assess client needs, monitor services, and contribute to program development and evaluation.
- Develop and implement program plans to meet department needs.
- Manage volunteers and staff, including orientation, training, and in-service sessions.
- Engage regularly with clients and health partners to ensure community needs are met.
- Serve as a primary contact between clients, health professionals, and community support.
- Track and report program outcomes and team efforts timely to supervisors and stakeholders.
- Support financial accountability by maintaining program budgets.
- Prepare financial and statistical reports for VON and funders.
- Conduct Disaster and Emergency Drills per policies.
- Participate in multidisciplinary meetings as required.
- Follow quality and risk management frameworks and occupational health and safety legislation.
- Participate in health and safety training, including WHMIS, and report incidents promptly.
External and Internal Relationships:
- Communicate promptly with clients and community partners.
- Maintain effective communication with internal staff.
Education, Designations, and Experience:
- Post-secondary diploma in Health/Social Science or equivalent.
- 2-3 years of community and health-based care experience.
- 1-2 years of program planning and evaluation experience (asset).
- Experience or understanding of not-for-profit healthcare environment (asset).
Skill Requirements:
- Strong facilitation and training skills.
- Completion of Fundamentals of Hospice Palliative Care (training provided).
- Experience in hospice palliative and grief/bereavement care (preferred).
- Experience with risk assessments.
- Education in Thanatology or grief and bereavement care (preferred).
- Ability to handle challenging conversations.
- High accuracy and attention to detail.
- Maintain confidentiality and discretion.
- Knowledge of social and healthcare services and community resources.
- Excellent communication skills.
- Ability to work independently and in a team.
- Strong organizational and multitasking skills.
- Proficiency in Windows OS and MS Office.

- Personal vehicle, valid driver’s license, and insurance.
- Satisfactory Criminal Records Check (original copy required).
- Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
- Ability to speak French is an asset in French-designated areas.
- Ability to wear PPE.
Working Conditions and Physical Demands: This role requires attention to detail in a dynamic environment, involving physical activity such as lifting, carrying, bending, reaching, kneeling, and other movements emphasizing good body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine hand movements throughout the day.
