The London Family Health Team (LFHT) is seeking a highly organized, resourceful and detail-oriented Registered Nurse to join our team at the London Beaverbrook Site in London, Ontario. This full-time role offers diverse clinical settings and opportunities to expand your professional skills while delivering high-quality patient care.

As a member of LFHT, you will collaborate with interdisciplinary teams, perform comprehensive nursing assessments, and provide education and care coordination.

Qualificações

• Current registration as a Registered Nurse with the College of Nurses of Ontario (CNO).
• Diploma or degree in Nursing; BScN preferred.
• Primary care, community, acute care, or related clinical experience.
• Strong clinical assessment, critical thinking, observation, problem-solving, and decision-making skills.
• Excellent professional, interpersonal, therapeutic, and patient/family communication skills.
• Demonstrated ability to provide effective patient and family health education.
• Demonstrated competence in nursing procedures appropriate to the role and scope of practice.

Responsabilidades

• Provide comprehensive, patient-centred nursing care across LFHT programs and clinical settings.
• Practice to full scope of practice, per CNO standards and policies.
• Complete nursing assessments, interventions, education, counselling, follow-up, and care coordination.
• Support patients with acute, chronic, complex, and preventive health needs.
• Provide nursing procedures and treatments within scope and demonstrated competency.
