Southern Health-Santé Sud in Morden is seeking a Shared Care Counsellor - Adult to provide one-time consultations and short-term counseling within Primary Care settings, collaborating with family physicians and consulting psychiatrists.

The role supports continuity of care, offers education and service coordination, and involves up to 6-8 sessions in a framework of advanced clinical practice, with travel to regional facilities as needed.

Competenze

• Knowledge and understanding of range of mental disorders and co-occurring substance use disorders per DSM V, including evidence-informed psychotherapeutic interventions.
• Demonstrated familiarity with psychotropic medications commonly prescribed for acute mental disorders.
• Demonstrated knowledge of Recovery-Oriented principles and confidentiality requirements.
• Proficiency with MS Office suite and ability to prepare written communications.

Mansioni

• Provide one-time consultation and short-term counseling (6–8 sessions) within Primary Care settings.
• Collaborate with family physicians, consulting psychiatrists and other health care services to ensure continuity of care.
• Offer education, service coordination and liaison activities to support integrated care.
• Perform psychosocial assessments and provide evidence-informed counselling interventions.
• Travel to other regional facilities as duties require.
