Northern Health is seeking a registered Dietitian to join our Primary Care department. You will work with interprofessional teams to create and implement care plans, centered on the person and family goals within the Primary Care Home model.

The role emphasizes clinical assessment, treatment planning, and follow-up, using evidence-informed practice to achieve meaningful outcomes in physician practices, homes, and communities.

Qualifications

• Baccalaureate degree in nutrition and dietetics and an accredited internship.
• Registrant of the College of Health and Care Professionals of British Columbia.
• Two years of clinical nutrition experience in a health care setting.

Responsibilities

• Collaborate with interprofessional teams to develop a plan of care.
• Deliver health services coordinated through the Primary Care Home.
• Conduct clinical assessments, planning, interventions, and follow-up.
• Apply evidence-informed practice to improve outcomes.
