Job summary

Interior Health is seeking an experienced Primary Care Clinical Pharmacist to join the Kamloops Northshore Urgent & Primary Care Center Pharmacy Team in Kamloops, BC. You will provide clinical pharmacy services, develop patient-focused therapeutic plans, and collaborate with an inter-professional team to optimize medication management.

This is a Relief Full Time (1.00) opportunity, working Monday to Friday, 08:00 to 16:00, with coverage until the return of the incumbent.

Qualifications

• Pharmacist registration with College of Pharmacists of British Columbia is required.
• At least 2 years of related patient care experience is preferred.
• Valid Class 5 driver's license and access to a vehicle for business purposes.

Responsibilities

• Provide clinical pharmacy services to primary care patients within the network.
• Develop patient-specific therapeutic plans and monitor therapy for effectiveness, adherence, adverse events, and interactions.
• Collaborate with the inter-professional health team and communicate findings to patients and practitioners.

About the Job

The Primary Care Clinical Pharmacist provides clinical pharmacy services to patients of the primary care network. As a member of an inter-professional team this position works directly with patients and members of the primary care health team to develop patient-specific therapeutic plans based on chart review, patient information and assessment of clinical responses. Embracing a patient-centered approach, this position provides comprehensive medication management and pharmaceutical care by monitoring the medical condition(s) and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions and therapeutic, chemical and laboratory incompatibilities. The Primary Care Clinical Pharmacist identifies and resolves actual and potential drug therapy problems in collaboration with the patient and other members of the primary health care team and communicates pertinent findings, recommendations and plans to the patient and appropriate healthcare practitioners.

Qualfications

• Current registration as a Pharmacist with the College of Pharmacists of British Columbia;
• A minimum 2 years recent, related patient care experience, ideally in a team based primary care setting or equivalent; and
• A valid Class 5 drivers license and access to a personal vehicle for business related purposes.
