Overview

Clinical Pharmacist, Primary Care Network. Join Fraser Health Authority as a Clinical Pharmacist within the Lower Mainland Pharmacy Services for our Primary Care Network located in Chilliwack, BC.

The BC Ministry of Health is funding the Pharmacists in PCN program to integrate clinical pharmacists into primary care teams across health authorities. The onboarding is done in collaboration with health authority and community stakeholders. Successful candidates will work closely with patients and prescribers to perform assessments, make recommendations, and implement care plans. Self-motivation, excellent communication skills, a strong clinical background, and adaptability to process improvements are key assets.

Salary

The salary range for this position is CAD $51.37 - $64.19 / hour.

Job Summary

Situated in Chilliwack, BC, this position provides clinical pharmacy services to patients within the Primary Care Network. Working as part of an interprofessional team, you will identify and resolve actual and potential drug therapy problems through comprehensive medication management.

Responsibilities
  • Direct patient care: communicate with patients and the health care team to determine goals of therapy, identify drug-related issues, formulate and implement a pharmacy care plan, monitor outcomes, provide medication information, and support care transitions.
  • Use pharmacotherapy knowledge to respond to drug information requests and contribute to drug therapy guidelines.
  • Follow and reinforce medication use policies to ensure proper dosing, storage, handling, dispensing, and administration of medications and compounded sterile products.
  • Participate in process improvements to ensure safety, reports of adverse drug reactions and medication errors, and continual patient safety during transitions of care.
  • Contribute to published literature on evidence-based pharmacotherapy (as applicable).
  • Provide preceptorship and supervision to pharmacy students and residents.

Additionally, you will validate prescriptions and identify safety concerns, working with the health care team to resolve discrepancies and document discussions or modifications to medication therapy and dispensing.

Qualifications
Education and Experience
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Two (2) years' recent, relevant patient care experience, ideally in a team-based primary care setting or an equivalent combination of education, training, and experience.
  • Valid Class 5 driver\'s licence and access to a personal vehicle for business-related purposes.
Skills and Abilities
  • Demonstrated ability to complete initial and ongoing patient assessments with clinical and diagnostic reasoning.
  • Ability to teach patients and others one-on-one and in groups.
  • Ability to integrate best practice and current research evidence to support professional practice decisions.
  • Effective communication with patients, families, the public, medical staff, and the interdisciplinary team.
  • Conflict resolution, data analysis, time management, and teamwork skills.
  • Ability to promote patient-centred care, cultural sensitivity, patient advocacy, and social justice concerns.
  • Proficiency with Electronic Health/Medical Records and Windows-based programs; data analysis and reporting tools.
  • Physical ability to perform the duties of the position.
