The North East Local Health Integration Network is seeking a dedicated Physiotherapist in Timmins. This role involves working with patients to provide essential physiotherapy services, with a focus on patient-centered care. Ideal candidates will have strong collaborative and communication skills and be registered with the College of Physiotherapists of Ontario.

As a Physiotherapist (Status: Permanent, Full-Time; Location : Timmins;Start date: Immediately ) , you providephysiotherapy services to Ontario Health atHome patients to reduce, eliminate,or prevent physical disabilities/impairments and improve the patient’s functionalindependence and safety in their respective environment.
Whether you work in our office, in thecommunity, or a health care facility – you will play a lead role in providingconnected, accessible, patient-centered care – and be supported by ourcollaborative team that includes over 9,000 regulated health care and otherprofessionals.

What will you do?
· Respond to patient referrals from Care Coordinators to properlyadminister physiotherapy services .
· Assesspatient's physical abilities to determine the impact of the patient’s conditionon their ability to function in their environment.
· Analyze, interpret, develop, and implement treatment programsbased on assessment results.
· Oversee patients progress and adjust treatment programs whenneeded, to ensure therapy goals are met.
· Assign physiotherapy programming to a rehabilitation assistantbased on patient needs and supervise according to professional standards.
· Educate and train patients, their families, caregivers, and/orSDMs, on how to progress with the treatment plan.
· Link patients with other in-house services or outsideorganizations if needed.
· Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected.
· Accurately document and chart patients’ assessment results, goals,treatment plans, and interventions.

· Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
· Casemanagement experience or recent related community experience
· Abilityto speak French or another second language
