Summary - Bayshore Integrated Care Solutions

The Physiotherapist, under the direction of the Manager of Rehabilitation or Clinical Manager- Branch level, is responsible for completion of professional activities that include: assessment, treatment, education, development of home exercise programs, supervision of Rehabilitation Assistants (RAs), and management of clients requiring Physiotherapy intervention, in accordance with professional standards.

  • 16 week home rehab program with a focus on maintaining client’s safety and well-being in their homes utilizing a client-centered approach.
  • Assessment of client’s mobility in home settings. The PT will then develop and assign a home exercise program to an RA who then sees the client at home multiple times per week to administer the program. PT will then reassess at the mid-way point to evaluate any changes in the client’s mobility status and adjust the home exercise program as needed. A final PT discharge assessment occurs at the end of the 16 weeks to determine if ongoing PT is needed following our program.

  • Treatment of patients with varying diagnoses, abilities, and goals. Patients discharged are typically coming home from Geriatric Rehab, Stroke Rehab, Complex Continuing Care, or Transitional Care Units.

Clinical Responsibilities

  • Responsible for the assessment of clients, establishment and ongoing evaluation and revision of treatment programs, including goals and discharge plans.
  • Maintains current and accurate clinical records according to standards set by the College of Physiotherapists.
  • Documents assessment results, goals, treatment, client progress and discharge plans.
  • Collaborates with other members of the health care team as needed.
  • Communicates pertinent information and recommendations to clients / care providers.
  • Provides education regarding treatment strategies as needed.
  • Observes the performance of those assisting in the physiotherapy program and provides guidance as necessary.
Requirements

  • Maintain active good standing with the Provincial College of Physiotherapists.
  • Participate in ongoing internal and/or external education activities.
  • Adhere to Bayshore Healthcare / Bayshore Therapy & Rehab Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participate in proactive Health & Safety activities while performing all duties. Responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
  • Maintain confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Complete other tasks as requested.
Qualifications

Education

Masters Degree or recognized equivalent in Physiotherapy.

Current registration, and in good standing with, the applicable provincial College of Physiotherapists.

Experience

  • Prior experience with in home assessments and treatments.
