Primary Function or Purpose of Job or Position

Credit Valley Family Health Team (CVFHT) is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated record of accomplishment of impactful quality improvement activities. It is a not-for-profit organization that receives funding from the Ministry of Health and Long-Term Care and provides primary health care services to over 15,000 patients in Mississauga. The CVFHT also has a mandate to provide French language primary care services to the Francophone community.

The Physiotherapist, under the direction of the Executive Director, will be responsible to advise patients and their families on health care including health promotion, disease, illness, and accident prevention. This position will collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate patient care and support in consultation with individuals and their families. This role will also respect and protect the rights of patients, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity. The Physiotherapist must possess a degree or diploma in science, health sciences, or a field appropriate to the position and certifications or licenses appropriate to the position.

Specific Duties or Responsibilities

  • Advise patients and their families on health care, including health promotion, disease, illness and accident prevention
  • Collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate patient care and support in consultation with individuals and their families
  • Respect and protect the rights of patients, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, belief, religion and culture; their right to privacy and dignity
  • Use effective interpersonal skills to communicate complex and sensitive information
  • Provide a consistently high standard of care and support for patients
  • Provide direct and comprehensive care and assistance to patients with specialized and complex needs
  • Assess, evaluate, plan and revise patient care / support
  • Ensure commitment to quality, patient safety and risk management initiatives
  • Provide education and counselling to patients and families, as well as provide ongoing advocacy and support to patients
  • Provide timely and astute information to other members of the team, as well as to individuals under direct care or support and their family members
  • Demonstrate cooperation among other staff members to ensure efficiency of care and support for patients
  • Protect the health and safety of others by adopting safe work practices and reporting unsafe conditions immediately
  • Perform responsibilities of the position within the legislative and regulatory standards set out in the applicable federal, provincial, and municipal legislation
  • Prepare and submit accurate reports, records, and charts
  • Degree or diploma in science, health sciences, or a field appropriate to the position
  • Certifications or licenses appropriate to the position
  • Valid first aid or CPR certification
  • Proficiency in EMR system is highly preferred
  • Previous experience in Family Health Team or in Primary Care is preferred
  • Ability to speak / read in French is considered an asset.

  • Demonstrated time management skills
  • High flexibility with strong interpersonal skills that allow one to work effectively in a diverse environment
  • Proven knowledge of case management processes and procedures
