The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care, and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home.

JOB SUMMARY:

We are hiring dynamic, passionate Physiotherapists who want to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will work in an environment that emphasizes professional autonomy within a self-directed team, serving a diverse client roster in the community. Your role involves collaborative care planning with a focus on client-centered goals, addressing physical, cognitive, social, emotional, and spiritual domains to create holistic care plans. Each client is assigned a primary care clinician who leads the care planning and facilitates the interdisciplinary team to reach the client’s goals. As part of the SE Transitions team, you will be accountable for the care and services you provide and the outcomes your clients achieve.

Requirements:
  • Graduate of an accredited Physiotherapy program and in good standing with the College of Physiotherapists of Ontario
  • Recent experience in acute/rehab or community-based rehabilitation preferred
  • Reliable, flexible, creative, self-directed, and able to work effectively with rehab assistants and interdisciplinary teams
  • Knowledge of community resources is an asset
  • Strong interpersonal, organizational, and communication skills
  • Positive work and attendance record
  • Ability to adapt to new computer applications
  • This role follows a hybrid work model, involving remote work and client visits in acute/rehab or community settings
  • Valid driver’s license and ability to travel
