Job Description
Summary

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities
  1. Evaluate the patient's functional status, all relevant body systems for CMS documentation, and physical therapy needs. Consult with the physician to develop the therapy plan of care.
  2. Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  3. Observe and report the patient's response to treatment and any changes in condition to the nurse supervisor and/or physician.
  4. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using evidence-based practices.
  5. Instruct and inform the healthcare team, patient, and family/caregivers about the plan of care, limitations, and progress toward goals.
  6. Visit patients within their service area and nearby agencies as needed.
  7. Complete progress notes for each visit and submit them according to policy.
  8. Complete evaluations and develop the PT plan of care within state guidelines. Report outcomes, goals, and projected care frequency within 24 hours of evaluation.
  9. Participate in staff meetings and committees as required.
  10. Supervise home health aides and PTAs as allowed by state regulation and agency policy.
  11. Participate in weekend rotations to provide services as needed.
  12. Prepare discharge summaries or condition reports at least every 60 days.
  13. Provide in-service education as requested.
  14. Adhere to therapy standards of practice and maintain up-to-date clinical knowledge and licensure.
  15. Perform other duties as assigned.
Education and Experience

Licensure Requirements

  • Current Physical Therapy license in Florida
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
