What you will do:

Responsibilities listed include but are not limited to:

1. Perform evaluations and develop treatment plans.
2. Regularly re-assess the effectiveness of treatment plans, and attend staffing meetings and family conferences.
3. Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
4. Communicate the patient’s needs and progress to the treatment team, physician, person receiving services, and family members.
5. Communicates with other disciplines to ensure collaboration, and coordination of care and enhance patient outcomes.

Qualifications:

What you will need:

1. Minimum of bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
2. Current, unrestricted license as Physical Therapist by state in which practicing.
3. Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
4. A minimum of one year’s work experience as a physical therapist is preferred.
5. Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
6. Communicates effectively and professionally in verbal and written interactions.
7. Ability to lift 50 pounds. Moving, lifting, or transferring of patients may involve lifting up to 100 pounds following safety procedures.
8. Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
9. Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times.
