Job description

Your Opportunity:

The Pharmacist I will provide direct patient care in accordance with the AHS Pharmacy Services clinical expectations standards at the University Hospital, Stollery Children’s Hospital, or Mazankowski Alberta Health Institute. This includes managing medication therapy for AHS patients collaboratively with other healthcare providers in a patient-centered model to ensure medications are used safely, effectively, and appropriately. You will assess patients for potential or actual drug-related problems and resolve or prevent them to optimize patient outcomes. The role involves active participation in patient care rounds, liaising with healthcare team members, developing professional relationships with patients and their families, and documenting all activities in the patients' Health Record. You will also participate in organizational practices, drug stewardship, quality improvement initiatives, and may incorporate additional prescribing authorization where practice allows. Maintaining professional development, precepting, and mentoring staff and students are also key responsibilities. Additionally, all pharmacists will perform dispensary duties, including medication order assessment, verification, and managing specialized medications, alongside providing drug information support.

As a Pharmacist I, you will be responsible for the safe, effective, and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and ongoing, and provide assessment, intervention, and resolution of drug-related problems, overseeing compounding and packaging activities.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

Ability to use computer medication order entry systems, access drug information resources, and proficiently use word processing. Fluency in written and verbal English. Ability to incorporate AHS organizational values into daily work. Ability to perform duties as outlined on the Physical Demand Statement (PDS). Ability to safeguard AHS information as private and confidential. Ability to follow policies, procedures, goals, and objectives. Effective communication skills. Proven initiative and motivation. Preferred Qualifications:

Experience with medication dispensing cabinets/unit dose drug distribution preferred. Additional Prescribing Authorization and injection administration preferred. PRAC ID preferred. IV additive, TPN, and chemotherapy certification preferred. Completion of an accredited hospital residency or PharmD program preferred. Experience in a large acute care hospital delivering proactive, integrated clinical practice (at least 1 year within the last 3 years) preferred.
