Position Overview: As a member of the pharmacy team, you are responsible for medication therapy management, pharmaceutical care, teaching students and staff, supervising technical personnel, and distribution activities.

Available roles include: Antimicrobial stewardship and med/surg float.

Primary Responsibilities
Pharmaceutical Care/Clinical Services:
  • Provide direct patient care to manage drug therapy, including electronic verification, patient education, medication reconciliation, and drug information.
  • Support drug distribution activities, including after-hours coverage.
  • Participate in educational programs and develop standards and protocols for pharmacy practice.
Educational Services/Professional Development:
  • Conduct inservice lectures, provide drug information, and participate in pharmaceutical care development.
  • Assist with clinical presentations, precept pharmacy students, and maintain continuing education.
Drug Distribution & IV Services:
  • Supervise pharmacy assistants, manage inventory, and ensure timely medication delivery.
  • Oversee IV admixture services, monitor TPN therapy, and participate in quality assurance.
Administrative & Other Duties:
  • Report medication discrepancies, participate in adverse drug reaction reporting, and train staff.
  • Participate in hospital committees, safety programs, and workload activities.
  • Attend departmental meetings and work flexible shifts as needed.
Qualifications
  • Bachelor or Doctor of Pharmacy, registered in Ontario, with certification from the Pharmacy Examining Board of Canada.
  • Hospital pharmacy residency or equivalent experience preferred.
  • Experience in clinical hospital pharmacy, especially oncology, is an asset.
  • Excellent communication, leadership, and organizational skills.
  • Knowledge of pharmacy systems like CPOE, eMAR, Cerner preferred.
  • Membership in CSHP or similar is an asset.
