Job summary

University Health Network (UHN) invites applications for a Personal Support Worker in the Spinal Cord Rehab Program at the Toronto Rehab Institute Lyndhurst Centre. This permanent part-time role is 0.4 FTE with shifts days and evenings, and a wage range of CAD 31.70 to CAD 32.61 per hour, start rate per collective agreement.

You will join an interprofessional care team to support daily living activities, assist with transfers, bathing and hygiene, and follow a prescribed Plan of Care.

Qualifications

  • Current registration with Health and Supportive Care Providers Oversight Authority (HSCPOA).
  • Current Basic Cardiac Life Support (BCLS) certificate.
  • Behavioral management training (PIECES, U-First, CPI) is an asset.
  • Ability to handle daily mild to moderate physical exertion.
  • Acute care and/or rehab experience is an asset.
  • Good attendance record is required.

Responsibilities

  • Assist patients with activities of daily living, including feeding, lifts/transfers, bathing, skin care, oral hygiene, and toileting.
  • Perform delegated tasks under RN/RPN supervision in accordance with RHPA after appropriate training.
  • Communicate and demonstrate basic information per the Plan of Care.
  • Ensure a safe environment by reporting unsafe conditions and patient changes.
  • Complete and maintain patient records and progress notes as per hospital policy.

Position Summary

The Personal Support Worker (PSW) is a member of the interprofessional care team and is expected to maintain a collaborative and active working relationship with team members in the provision of patient care.

Duties

  • Assistpatients with activities of daily living: This includes feeding, lifts and transfers, bathing, skin care, oral hygiene, and toileting.
  • Perform tasks assigned or as delegated by an RN or RPN in compliance with the RHPA for whichappropriate educationand training has been completed. RNs and RPNs willbe responsible forthe oversight of the PSW when performing the assigned or delegated tasks. Delegated tasks can include actions that involve insertion of a digit or instrument into a body cavity, or any care or procedure under the dermis, for example.
  • Communicate anddemonstratebasic informationin accordance witha patient’s pre-established Plan of Care.
  • Ensure a safe environment for patients and staff by reporting and documenting unsafe conditions such as broken equipment as well as any patient behavioral, physical, or cognitive changes. Safety concerns must be reported to an RN, RPN or other regulated health provider, in compliance with patient safety responsibilities and corresponding hospital plans, policies and proceduresin order toensure a safe environment for patients.
  • Complete andmaintainrelated patient records and progress notes, as per hospital policy.
    • Current registration with the Health and Supportive Care Providers Oversight Authority (HSCPOA);
    • Current Basic Cardiac Life Support (BCLS)Certificate;
    • Behavioral management training (such as PIECES, U-First, CPI) is considered anasset
    • Ability to handle daily mild to moderate physical exertion (i.e.constant standing, walking, climbing, reaching and frequent movement) on an occasional to frequent basis;
    • Acute care and/orpreviousrehab experience considered anasset;
    • Good attendance record isrequired
