Extendicare in Canada is seeking a dedicated Personal Support Worker (PSW) to provide direct personal and nursing care to residents under the direction of Registered Staff and the Director of Care.

The role emphasizes a zero lift policy and supports residents with ambulation, feeding, bathing, grooming, meal service, and participation in activity programs, while documenting care, following policies, and maintaining safety and infection control in a compassionate, team-driven environment.

Responsibilities

• Assists in providing direct care for residents; assists with ambulation, feeding, bathing and grooming.
• Encourages participation in activity programs by assisting residents to and from activities.
• Assists with meal service and nourishment delivery to residents and provides feeding assistance as required.
• Provides emotional and social support to residents/families including palliative care.
• Adheres to established departmental polices and procedures regarding documentation, quality assurance, fire, safety, environment and infection control.
• Consults with and informs supervisor of unit activities, needs and matters related to resident care.
• Consults with and informs team leader of activities, needs and problems related to resident care. Utilizes observation skills to note changes in resident’s condition. Keeps team members informed of observations through Point of Care Charting.
• Participates in care conferences and documentation related to resident care plans.
• Assists with admission, discharge and transfer of residents.
• Adheres to established department policies and procedures regarding quality assurance, fire, safety, environment and infection control.
