The Personal Support Worker (PSW) reports to the Director of Care and, under the direction of the Unit Supervisor, provides personal care, reports changes in resident status, and documents where applicable. The role involves enhancing restorative care by promoting independence in activities of daily living, improving therapeutic bathing effects, and creating a supportive environment tailored to residents’ needs.

Responsibilities

• Assist residents with personal care and activities of daily living as per their care plans, promoting independence and self-determination.
• Monitor residents’ wellbeing and report physical or emotional changes to registered staff.
• Accurately document care activities in electronic health records per policies.
• Follow all policies, procedures, and the Code of Conduct.
• Maintain effective communication with residents, families, and coworkers.
• Support the organization’s vision, mission, and residents’ rights.
• Ensure health and safety standards are met for residents, families, and staff.
• Participate in care conferences and team meetings, contributing to care plan development.
• Assist with Activities of Daily Living (ADLs) and monitor safety according to care plans.
• Document all care activities thoroughly and promptly.
• Maintain a clean, safe, and secure environment during care provision.
• Assist with meal service and feeding as needed.

Qualifications

• PSW Certification from a program of at least 600 hours, including practical experience.
• The program must meet Ministry of Colleges or Universities standards for Ontario.
• Experience with person-centered care models.
• Excellent communication and interpersonal skills.
• Ability to turn tasks into positive social interactions.
• Legal requirements including a current Vulnerable Sector Check and TB testing results.
• Two supervisory references.
• Proof of required vaccines; staying up-to-date is recommended.
