Job Summary:

You will provide companionship, personal care, and health maintenance services to clients in their homes or community, aligned with VON’s philosophy, policies, standards, and legislation. Collaborate with the Health Care and Personal Support Teams to support clients in achieving independence and well-being.

This position is eligible for a $3.00/hour wage enhancement from the Ontario Government for hours spent directly caring for clients.

Foreign Registered Nurses awaiting CNO registration and PSW students nearing graduation are encouraged to apply.

Responsibilities:

• Monitor the individual's home and personal situation to identify needs for support or professional assistance.
• Assist in developing and implement the personal support care plan, including promoting self-care and independence, providing personal care, advocating for clients, and evaluating care effectiveness.

Qualifications:

• Mandatory: Personal Support Worker certification or equivalent education (DSWs, HSWII, HSWIII, HCA)
• Completion of Gentle Persuasive Approaches course is an asset
• Preferred: Fundamentals of Palliative Care - Level I
• Current CPR and First Aid certifications
• Experience with seniors, pediatrics, mental health, cognitive impairments, obstetrics, palliative/oncology, or ill individuals is an asset, including Alzheimer’s or dementia care
• Household management experience and ability to adapt to various social and economic environments
• Knowledge of community resources, strong communication, critical thinking, assessment, and problem-solving skills
• Ability to work independently and in multidisciplinary teams, willingness to travel, reliable transportation, valid driver’s license, and proof of vehicle insurance
• Criminal Records Check and Vulnerable Sector Search required; ability to speak French is an asset in designated areas
