Job summary

Jarlette Health Services invites applications for a Personal Support Worker (PSW) at Meadow Park Chatham, on-site in Chatham, Ontario. This part-time casual role supports residents with activities of daily living under the direction of the Co-Director of Care.

The PSW will provide direct patient care, monitor health changes, complete documentation, and work closely with an interdisciplinary team while maintaining a safe, respectful environment. Vaccination/testing requirements apply.

Qualifications

• Must have completed a PSW program meeting provincial requirements and provide proof of graduation.
• Candidates enrolled in an RN or RPN program in Ontario will be considered.
• Experience in Long Term Care (Gerontology) is an asset.
• Must demonstrate good communication skills.
• Must demonstrate an active interest in ongoing education.
• Displays a caring and respectful attitude toward residents, families and team members.
• Understands the demands and rewards of the role.
• Must be computer literate.

Meadow Park Chatham is a fully accredited long term care home in Chatham, Ontario and serves 99 residents. Chatham is the urban centre of Chatham-Kent, a large municipality in Southwestern Ontario located between Windsor (approximately 80 km to the west) and London (approximately 115 km to the east). It is known for its fertile farmland, a diverse economy of manufacturing and agriculture, and rich history, including its role in the Underground Railroad.

Join our team as a Personal Support Worker (PSW)!

Under the direction of the Co-Director of Care, the Personal Support Worker (PSW) is an integral part of the health care team, and in this role, encourages, supports and promotes independence for the residents by assisting with activities of daily living.

The Personal Support Worker (PSW) is responsible for a comprehensive range of duties encompassing resident care, interdisciplinary collaboration, adherence to organizational policies, and the maintenance of a safe environment. Core responsibilities involve provision of direct patient care, diligent observation and timely reporting of changes in health status, and accurate completion of all required documentation.

We are looking for the following qualifications and traits:

• Must have successfully completed a personal support work program that meets the requirements in subsection 52. (2) of the Fixing Long Term Care Act, 2021 and Ontario Regulation ss. 246/22; and provides proof of graduation issued by the education provider
• Candidates are also considered who are enrolled in a RN or RPN program in Ontario
• Experience in Long Term Care (Gerontology) setting is an asset
• Ensures all Ministry of Long Term Care requirements are met
• Must demonstrate good communication skills
• Must demonstrate an active interest in ongoing education
• Displays a caring and respectful attitude toward our residents, families, and team members
• Understands the demands and rewards of the role
• To enhance the requirements for quality customer service and engagement
• Must be computer literate
