Job Summary:
The Personal Support Worker provides personal support and homemaking services to clients and families within the framework of VON policies, procedures, standards. The PSW collaborates with members of the health care team to meet clients' needs, promoting independence and well-being.
This position is eligible for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.
Key Responsibilities:
- Assess and evaluate clients' and families' home and personal situations to determine if adjustments to the care plan are needed, and report findings to the supervisor.
- Assist in developing the personal support care plan under the guidance of the Personal Support Program Manager, incorporating information from the funder, manager, and clients/families.
- Implement changes to the care plan for safety, liability, and financial reasons, under managerial guidance.
- Report any changes in client or family needs or home situation that may require adjustments to the care plan.
- Perform personal support tasks based on client and family needs, including light cleaning, meal preparation, clothing care, shopping, and household safety monitoring.
- Provide personal care as directed, including assistance with mobility, hygiene, dressing, and eating.
- Offer bedside care when required, under nurse supervision.
- Provide respite care and support to clients’ families, including child care if needed.
- Organize daily activities and prioritize tasks under managerial supervision.
- Complete daily call checks and other documentation to ensure service effectiveness.
- Assess clients' communication and relational abilities.
- Communicate with the health team regarding client needs and inform supervisors of any concerns.
- Comply with Occupational Health and Safety legislation, policies, and procedures.
- Follow risk management policies to promote client safety.
- Perform other duties as assigned.
External and Internal Relationships:
- Regular engagement with the supervisor or Program Manager regarding client plans and relevant updates.
Education, Designations, and Experience:
- High school diploma.
- CCA Certification required in Nova Scotia.
- Certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
- Current First Aid or CPR Level C certification, if applicable.
- Post-secondary degree or diploma is an asset.
- Experience in household management and working with elderly, ill individuals, or children.

Other:
- Must have access to a personal vehicle, valid driver’s license, and proof of insurance.

- Flexible availability and ability to wear PPE.
Working conditions involve physical activities such as lifting, bending, reaching, and walking, with an emphasis on body mechanics and safety.
