We are seeking a passionate Personal Support Worker (PSW) to join our team in providing client-centered home and community care. You will work within a self-directed, collaborative team that emphasizes professional autonomy and holistic care planning, focusing on physical, cognitive, social, emotional, and spiritual well-being. Each client is assigned a primary care clinician who leads their care planning, with the interdisciplinary team working together to achieve client goals.

Qualifications to work as a PSW:

• Valid PSW Certificate
• RN or RPN Nursing student (with 2 clinical placements completed)
• Valid Developmental Support Worker certification
• Foreign-trained healthcare professionals (Nurse or Physician)

Requirements:

• Valid driver’s license and vehicle
• Recent immunization record (TB test within past 12 months)
• Vulnerable sector check upon hire
• Availability for days, evenings, and weekends
• Ability to work independently and in teams, with strong organizational skills
