Job summary

The Northwest Territories Health and Social Services Authority (NTHSSA) seeks Personal Support Workers to deliver Home Care in Yellowknife, Dettah and N'Dilo. You will provide culturally sensitive ADL support to clients in their homes, typically visiting 4-5 clients per day and coordinating with a multi-disciplinary team.

You must have Grade 12 and a related program such as Long Term Care Aide or Personal Care Attendant, a valid Class 5 license, a clean driver’s abstract, and a successful RCMP

Qualifications

  • Grade 12 and completion of a Long Term Care Aide or Personal Care Attendant program and 3 months related experience in home care.
  • Valid Class 5 driver's license and a clean driver's abstract.
  • Satisfactory Vulnerable Sector Check with the RCMP.
  • Yellowknife regional training requirements: Aurora College Pharmacology Course; Simple Wound Care; Foot Care; Food Safety.

Responsibilities

  • Provide direct personalized care to Home Care clients.
  • Visit 4–5 clients per day.
  • Advocate for clients and facilitate communication with the health care team.
  • Collaborate with health care staff to support patient well-being.

Job Information

Personal Support Workers provide culturally sensitive, respite and ADL care to clients of the Home Care program in Yellowknife, Dettah and N'Dilo. The Personal Support Worker visits 4-5 clients a day. In most cases, the Personal Support Worker visits the client independently, working with minimal supervision. Clients of the Home Care Program are individuals of all ages who usually require ongoing care but are not at the point where they require 24-hour complete care. These clients include but are not limited to long-term convalescent, geriatric, respite, surgical or medical patients who live within their home and community and require scheduled treatment, care and/or support.

The Personal Support Worker (PSW) is a member of a multi-disciplinary team who provides direct personalized care to residents who have common well-defined nursing diagnoses. The PSW acts as a client's advocate and facilitates communication between the client, family, Home Care Nurses, and other health care professionals. This results in a holistic approach to planning, organizing, teaching, and relationship development that will best meet the needs of the residents.

Knowledge, Skills and Abilities
  • Knowledge of and an ability to apply basic first aid and CPR in order to preserve life and promote healing
  • Knowledge of healthy aging process
  • Knowledge of traditional healing methods as well as community and cultural norms
  • Sensitivity to geographical and cultural diversity
Typically, the above qualifications would be attained by:
  • This level of knowledge is commonly acquired through the successful completion of grade 12 and the successful completion of a Long Term Care Aide or Personal Care Attendant program and 3 months directly related, recent, experience in a home care setting or other health care setting as a home support worker or personal care attendant.
Requirements:
  • The incumbent must have a valid class five driver's license,
  • Provide a satisfactory driver's abstract
  • Complete a satisfactory Vulnerable Sector Check with the RCMP

Within the Yellowknife Region, HSW's must be able to acquire within a reasonable time frame and remain current with the following training and certifications:

  • Aurora College Pharmacology Course
  • Simple Wound Care (In House)
  • Foot Care (In House)
  • Food Safety (Environmental Health Officer)
Directly Related Experience:
  • Assist with activities of daily living
  • Assess and record a client's change in medical condition
  • Advocate the dignity and self-respect of clients
  • Participate in case conferences concerning client concerns or progress
  • Collaborate with health care team for the health and well-being of patients/clients and staff
