Job summary

Maamwesying North Shore Community Health Services is seeking a Personal Support Worker to join our Home and Community Support Services team. The role provides in-home care to Garden River First Nation and Batchewana First Nation, under supervision of the Home Care Service Manager.

You will work within an interdisciplinary, culturally grounded care environment that values holistic wellness, relationship-building, and respectful, community-centered service.

Qualifications

• PSW certificate from an accredited program; relevant community/home care experience is an asset.
• Strong problem-solving, judgement and decision-making abilities.
• Ability to prioritize tasks, manage multiple clients, and organize workload.
• Excellent interpersonal and collaborative skills.
• Experience with electronic charting is an asset.
• Valid Ontario driver’s license and reliable vehicle; willing to provide driver’s abstract.

Responsibilities

• Provide in-home care to Garden River First Nation and Batchewana First Nation.
• Ensure a continuum of care per the care plan for vulnerable seniors.
• Collaborate within an interdisciplinary and culturally grounded care environment.
• Adhere to Home and Community Support Services guidelines.

Salary: $23.34-$27.06/hour (commensurate with experience)

The opportunity: Maamwesying North Shore Community Health Services provides primary health care, traditional healing, mental wellness and addictions, home and community support services, and health promotion services to community members across the seven First Nations of the Mamaweswen North Shore Tribal Council.

We are currently seeking a highly skilled and compassionate Personal Support Worker to join our Home and Community Support Services team to provide in-home care to Garden River First Nation and Batchewana First Nation.

Under the direction of the Director of Home and Community Support Services, and the clinical supervision from the Home Care Service Manager for the Assisted Living Services, this position is responsible to ensure a continuum of care for vulnerable, high-risk seniors as per the care plan. This position offers the opportunity to work collaboratively within an interdisciplinary and culturally grounded care environment that values holistic wellness, relationship-building, and community-centered care.

Qualifications & Experience

• Personal Support Worker Certificate from a recognized accredited program; with relevant experience in working in a community-based and home care setting is an asset.
• Problem-solving, good judgement and decision-making skills and ability;
• Ability to prioritize professional duties, manage multiple patients, and efficiently organize workload;
• Strong interpersonal and collaboration skills;
• Experience in electronic charting is an asset;
• Ability to work independently and within a multi-disciplinary team;
• Must possess a valid Ontario Driver’s License and a reliable vehicle and be willing to provide a driver’s abstract.
