Job summary

Alberta Health Services is seeking a Licensed Practical Nurse to serve as a Personal Support Coordinator in Elk Point Healthcare Centre, working within Home Care and the Long Term Care multidisciplinary team. This 0.60 FTE temp PT role focuses on patient- and family-centered care, safety, and effective communication with clients, families, and health professionals.

The position requires LPN licensure, BCLS-HCP, at least 2 years in seniors care, and leadership experience to mentor Health Care

Qualifications

  • Completion of an accredited Licensed Practical Nurse education program.
  • Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
  • Minimum 2 years' recent experience as an LPN in Seniors health or long-term care.
  • Previous experience supervising or leading staff.
  • Experience providing direction, teaching, mentoring, or coaching Health Care Aides.
  • Demonstrated experience working within an interdisciplinary team environment.

The Elk Point Healthcare Centre has an opportunity for a passionate Personal Support Coordinator (PSC) dedicated to patient- and family-centered care. This role will be an integral part of the multi-disciplinary team in Long Term Care which is made of 30 beds. You will build meaningful relationships as you work with clients daily providing personal assistance and support services to those in your care. Superior interpersonal skills will be used daily as you work closely with clients, families, caregivers, and health professionals. You will help maintain a safe environment, observe and report on patients' health and well-being, and ensure effective communication. Your role includes direct client care demonstrating genuine interest and respect for clients and families in your care. As a Licensed Practical Nurse (LPN) Personal Support Coordinator, within the Home Care environment, you are part of an interprofessional health care team responsible for the planning, organizing, implementing, monitoring and evaluating of personal support services. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
