Job summary

Alberta Health Services in rural east-central Alberta is seeking a Licensed Practical Nurse to provide community-based care in the County of Flagstaff, reporting to the Care Manager and supervising Health Care Aides. You will work independently in client homes, lift and carry supplies, and engage in wound care, palliative care, and patient education.

In this role, you will utilize your strong oral and written communication and deliver quality nursing care to clients in all developmental stages in the community. Utilizing your experience in wound care, palliative care and client teaching, you will take a collaborative approach to meeting health goals, contribute to the nursing diagnosis and participate in nursing interventions and outcomes. Required Qualifications:

• Completion of an accredited practical nurse education program.

Additional Required Qualifications:

• Valid Class 5 Driver's License (GDL license is not acceptable).
• Must be able to meet the physical demands of the position: This position requires frequent physical demands including walking, bending, lifting, and standing and driving for long periods of time. Transferring patients of varying sizes and degrees of mobility. Transporting supplies and equipment. Driving in all weather conditions.

Preferred Qualifications:

• Appropriate competency in Meditech, Care Manager and RAI-HC programs and knowledge of Outlook and MS Teams.
• Connect Care training/experience.
• Broad knowledge of provincial, regional, and local community resources.
• Minimum 1 year previous Home Care and/or geriatric experience.
