About the Role:

You will work with a team in our Community Access program to support individuals with disabilities, some with complex behaviors and needs. The team is fast-paced, requiring adaptability and situational awareness. Daily activities include attending, participating, or engaging in various activities with the individuals supported. You will spend time in the community, outdoors, driving to meet individuals, or transporting them for appointments or activities across the city. Your focus will be on supporting clients' life skills and goals. Individuals supported may have developmental delays, Autism, ADHD, Borderline Personality Disorder, Cerebral Palsy, Schizophrenia, or other mental health or developmental conditions.

Daily responsibilities may include, but are not limited to:

  • Providing medical, technological, environmental, emotional, physical, and behavioral support
  • Attending medical and legal appointments with individuals
  • Completing documentation, medication administration, and key worker duties
  • Assisting with daily chores and activities in the community or group home
  • Supporting safety and cleanliness of living spaces
  • Performing tasks such as fire drills and hazard assessments
  • Communicating with OPG/Guardians/Family members
  • Collaborating, advocating, and researching support options, including hospital visits if needed
  • Providing specific support for individuals on ODP or O.A.T programs
  • Transporting individuals to activities or appointments throughout the day
  • Learning behavioral support plans to understand individual needs
  • Completing incident reports accurately and promptly

This is a part-time permanent position, 24 hours/week, with a high potential for extension. The schedule is Monday to Friday, 9:00 a.m. to 5:00 p.m. The starting pay rate is $21.19/hour, with potential for raises after the annual evaluation. After a three-month probation, you will be eligible for health and dental benefits, along with 5 flex days. Mileage reimbursement is provided at $0.50/km, as driving is required.

Minimum Requirements:
  • A Certificate/Diploma in Human Services or Disability sector (equivalencies considered)
  • Valid Class 5 driver’s license, reliable vehicle, and minimum $2,000,000 liability insurance by time of hire
  • One year of relevant experience in human services or disability sector
  • Comfortable supporting individuals in a swimming pool
  • Support individuals with diverse disabilities and complex diagnoses
Preferred Requirements:
  • Training or experience with lifts, transfers, or g-tubes
  • Comfortable supporting individuals around animals/pets
  • Experience working with families, community resources, and building stakeholder rapport
  • Experience supporting individuals with mental health illnesses, ASD, or addictions is an asset
