The Opportunity

The Social Worker reports to the Director of Care and collaborates within the interdisciplinary team to deliver comprehensive care. Responsibilities include psychosocial assessments, counseling, legal/financial assistance, and community referrals, utilizing a holistic approach.

  • Conduct comprehensive psychosocial assessments for new residents, including psychological, social, and spiritual well-being.
  • Establish rapport with residents and families through initial interviews and ongoing engagement.

What You Will Need
  • Registration as a Social Worker under the Health Professions Act, 2000.
  • Bachelor of Social Work degree from a recognized institution.
  • Experience working with geriatric populations.
  • Familiarity with multidisciplinary team dynamics and effective teamwork skills.
  • Ability to initiate and manage departmental activities independently.
  • Current Vulnerable Sector Check (or willingness to obtain one).
