Job summary

Sinai Health in Toronto is seeking a Social Worker for the Family Medicine team at Mount Sinai Hospital Campus. You will assess psychosocial needs, provide counseling, crisis intervention and coordinate community referrals for perinatal populations.

You will work with an interdisciplinary team, contribute to learner education, and support mental health care planning and program development within the MSAFHT.

Qualifications

• MSW or equivalent required.
• Registered with Ontario College of Social Workers and Social Service Workers in good standing.
• Minimum 3 years direct client service experience in community health care.

Responsibilities

• Assess and diagnose psychosocial problems.
• Provide individual, couple, family, and group counseling.
• Manage cases with linkages to community resources.
• Conduct psychoeducation for mental health issues and group sessions.
• Navigate service networks to secure resources for patients.
• Advocate for access to needed resources.
• Support patient self-management with education resources.
• Contribute to learner education in an academic setting.
• Implement standardized screening tools for team delivery.
• Develop and deliver programs solo or with others.
