Job summary

Carlington Community Health Centre seeks a Health Promoter for its Perinatal programs to lead, coordinate and evaluate services that improve outcomes for pregnant individuals, infants and families.

The role requires strong health promotion expertise, group facilitation, and partnership development to address social determinants of health in Ottawa. Part‑time 0.8 FTE, reporting to the Director of Healthy Growth and Development.

Qualifications

• Bachelor’s degree in Health Promotion, Public Health, Nursing, Community development, Social Sciences, Midwifery, or related discipline, or equivalent experience.
• Specialized education/training in perinatal health, maternal‑child health, breastfeeding support, early years, mental health promotion, or community development is essential.

Responsibilities

• Lead planning, delivery, monitoring, and evaluation of the Perinatal Parent Support Program.
• Facilitate groups, workshops, outreach, and health education activities; develop culturally responsive resources.
• Build coordinated service pathways with partners and support student/volunteer learning opportunities.
• Support program planning, daily operations, referrals, waitlists, and service flow.
• Provide onboarding, mentorship, and team learning opportunities for staff and volunteers.
• Oversee quality assurance, documentation standards, reporting and continuous improvement.

Carlington Community Health Centre is a non-profit organization committed to improving health and well-being through quality health and social services for families, youth, adults, and seniors. Carlington Community Health Centre is seeking a Health Promoter for its Perinatal programs who will be responsible for the development, coordination, implementation, monitoring, evaluation and continuous quality improvement of perinatal programs and services. The role serves as a resource to parents and a mentor to team members by promot ing evidence-informed practice that improves outcomes for pregnant individuals, infants, parents and families.

The role is part-time (0.8) and reports to the Director of Healthy Growth and Development.

Key Responsibilities

Program Functional Leadership, Coordination and Service Delivery

• Leads the planning, delivery, monitoring, and evaluation of The Perinatal Parent Support Program, using community needs, evidence, data, and feedback to guide improvements.


• Facilitates groups, workshops, outreach, and health education activities; develops culturally responsive resources that promote health equity, family well‑being, and community capacity.


• Builds coordinated service pathways with community partners, contributes to funding and reporting activities, and supports student and volunteer learning opportunities.

Operational Leadership, Quality Assurance and Staff Support

• Supports the Director with program planning, annual objectives, and daily operations, including referrals, waitlists, client prioritization, and service flow.


• Provides functional leadership, onboarding, mentorship, consultation, and team learning opportunities for staff, students, and volunteers.


• Oversees quality assurance, documentation standards, performance monitoring, data reporting, and continuous quality improvement

Team related activities

• Works with the Healthy Growth and Development Department and interdisciplinary teams to support coordinated, integrated services.


• Participates in meetings, committees, professional development, organizational initiatives, and strategic planning.


• Builds partnerships, represents the Centre on relevant planning groups, and strengthens referrals and collaborative responses to family needs and social determinants of health.

Networking and Community Partnership Activities

• Maintains active relationships with community agencies, CHCs, schools, hospitals, Ottawa Public Health, and other service providers.


• Represents the Centre at coalitions, working groups, community meetings, events, and city‑wide planning tables related to perinatal, early years, and maternal‑child health.


• Promotes available services, strengthens community referral pathways, and supports coordinated approaches to improve family access and well‑being.


• Extensive knowledge of Ottawa’s perinatal, infant, early years, and community support services, with a strong understanding of health promotion, population health, and the social determinants of health.


• Demonstrated commitment to equity, anti‑oppressive practice, trauma‑informed care, and culturally responsive service delivery.


• Strong interpersonal, communication, facilitation, relationship‑building, leadership, mentorship, consultation, and community engagement skills; able to work effectively with interdisciplinary teams and community partners.


• Knowledge and experience in group facilitation, adult education, community capacity building, program delivery, and service coordination.


• Strong analytical, organizational, problem‑solving, program planning, project coordination, quality improvement, and outcome‑measurement skills; able to collect, analyze, interpret, and report program data to support planning and decision‑making.


• Ability to work independently, manage multiple priorities, and support operational planning in a dynamic environment; proficient with Microsoft Office, Zoom and other virtual meeting platforms, electronic documentation systems, Canva, and social media tools.

Qualifications

Education

• Bachelor’s degree from a recognized university in Health Promotion, Public Health, Nursing, Community development, Social Sciences, Midwifery, or a related discipline; or an equivalent combination of education and significant related experience.


• Specialized education or training in perinatal health, maternal‑child health, breastfeeding support, early years, mental health promotion, or community development is essential.

Experience

• Minimum of five years of progressive experience in health promotion, maternal‑child health, family support, community development or related community‑based service


• Experience planning, delivering, monitoring and evaluating programs; facilitating groups and health education; and supporting quality improvement and performance measurement


• Demonstrated leadership experience, including program coordination, project leadership, mentorship, staff orientation, and collaboration with interdisciplinary teams.


• Experience working with diverse and marginalized communities, conducting outreach, building community partnerships, advancing health equity, and connecting families to services, non‑profit/community health experience and grant‑writing experience are assets.

Languages

• English and French Fluency essential


• A third language relevant to the community served is considered an asset
