Job summary

Sara Riel Inc. in Winnipeg is seeking a dedicated Housing and Community Supports professional to assist participants experiencing homelessness.

You will connect individuals to resources, support services, and work toward securing permanent housing in a bilingual capacity. Reporting to the Manager of Tenancy Supports and System Navigation, you will assess functioning and mental health, document rehabilitation plans, conduct intake interviews, and provide person-centered housing and community

Responsibilities

  • Administration — complete all administrative duties including database, research, and reports; ensure documentation adheres to PHIA, FIPPA and confidentiality standards
  • Demonstrate effective communication and maintain regular collaboration with management, staff, participants, and referral sources
  • Assist service recipients in preparing for, finding, getting, and keeping housing in their community of choice; act as liaison between tenant and landlord
  • Provide support for mental health/addiction supports and person-centered recovery care
  • Document all relevant information and generate concise reports; gather and record statistical information for program delivery and evaluation
  • Participate in Sara Riel functions/events at least once per year and assist with planning and participation
  • On-site pay and benefits discussions; pay: From CAD 22.00 per hour and expected 40 hours per week

Job Summary

Reporting to the Manager of Tenancy Supports and System Navigation, the incumbent will be responsible for providing support to participants experiencing homelessness get connected to resources, and supportive services and in assisting them in achieving goals that will lead to successful community living and obtain permanent housing. The incumbent will be responsible for assessing individual’s level of functioning and mental health, developing, documenting, and implementing of individual rehabilitation plans and individual intake interviews. The incumbent will be responsible for all aspects of assisting participants with housing, community integration and employment. This is a bilingual position in both official languages.

  • Demonstrate a positive role model in all internal and external interactions, maintaining rapport with referral sources, to build a strong network of external resources, and facilitate intake
  • Provide feedback, reports, and data on progress of programs and participants
  • Demonstrates effective communication skills
  • Maintains regular communication and collaboration with management, staff, and participants
  • Seeks assistance as necessary.
  • Communicates in clear, respectful, and courteous manner with all staff, participants, stakeholders, and the community
  • Relays accurate program and agency information
  • Personal demeanor, professional behavior and staff/client relationships reflect mission, values, and moral philosophy of Sara Riel
  • Assist Service Recipients in preparing for, finding, getting, and keeping housing in their community of choice and act as liaison between tenant and landlord - by helping Service Recipients search for housing, understand their tenancy responsibilities, connect with the community, and navigate additional support
  • Assist with building independent living skills in house or once housing is secured.
  • Engages with tenants and follow through on inquires, requests, and complaints in a respectful and timely manner.
  • Work as part of a team to provide mental health/addiction supports to participants.
  • Demonstrate a positive role model in all internal and external interactions, maintaining rapport with referral sources, to build a strong network of external resources, and facilitate intake
  • Provide support and services under a person-centered recovery care model in accordance with to the self-identified needs of the Service Recipient.
  • Facilitate psycho-educational and peer support groups
  • Provide on and offsite support for our Youth Hub
  • Document all relevant information and generate clear, concise, and accurate reports detailing significant interactions; and provide oral summarizations at shift changes; gather and record all statistical information necessary for program delivery and evaluation.
  • Other duties as assigned and in accordance with outlined policies, procedures, and guideline
  • Participate in initiatives for Sara Riel functions/events at least once per year.
  • Support could in include planning, development, coordination and general participation with events.
  • Company events may take place both during and outside regular working hours. Employees participating in an event outside of working hours will be permitted to flex their time.

Other duties as assigned and in accordance with outlined policies, procedures and guidelines.

Pay: From $22.00 per hour

Expected hours: 40.0 per week
