Job summary

Saint Elizabeth in Ontario is seeking registered nurses or registered practical nurses to deliver community-based care. You will assess clients, develop and implement care plans, and collaborate with the care team to ensure high-quality, evidence-informed practice.

The role requires availability days and every other weekend, a valid CN nursing license, CPR, and willingness to obtain Saint Elizabeth certifications.

Qualifications

• Active registration with the College of Nurses of Ontario (CNO).
• Canadian Nurses Association specialty certification is an asset.
• Current CPR certificate is required.
• Valid driver’s license and vehicle (travel required).
• Up-to-date immunization records and clear background checks.
• Willingness to attain Saint Elizabeth certifications.
• Availability to work days and every other weekend.
• Ability to lift up to 55 pounds and perform required physical tasks.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best practices and client needs.
• When designated as primary provider, act as the care lead within the client caseload and collaborate with the care team.
• Support clients through transitions by connecting with community support services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current registration with the College of Nurses of Ontario
• Canadian Nurses Association (CNA) specialty certification an asset
• Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
• A current CPR certificate is required
• A valid driver’s license and vehicle (travel required)
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required Saint Elizabeth certifications.
• Availability to workdays and every other weekend.
• This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
