Qualifications

Pediatric Occupational Therapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark

Employment Type: Independent Contractor

Location: Currently hiring for Ajax, Durham Region, ON

Sign-on bonus up to $5000.00 available!

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Occupational Therapists to provide services to our pediatric clients in their school and homes within the community of Durham Region. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size.

OT Responsibilities:

  • Provide ministry-funded, community-based Occupational Therapy services within the pediatric client’s (infant to 18 years of age) place of residence, private school and/or through virtual services
  • Provide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of fine motor, sensory, feeding, developmental, home safety, home modifications, equipment prescription, mobility, etc.
  • Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers
  • Assign patient care plan, and provide supervision and monitoring of occupational therapy assistants when appropriate
  • Effectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriate
  • Complete funding applications as applicable
  • Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standards
  • Opportunity to service private clients

Requirements:

  • Registered Occupational Therapist with the College of Occupational Therapists of Ontario (required)
  • Experience working within a community setting is an asset but new graduates are welcome
  • Ability to work independently and as part of an inter-professional team
  • Ability to prioritize, manage self-scheduling and meet reporting deadlines
  • Excellent oral, written and report writing skills
  • Proficiency with computers
  • Strong interpersonal and organizational skills
  • Reliable transportation is required for this position
