Qualifications

• 1 year LTC experience and 1–2 years clinical experience preferred.

Responsibilities

• Provide compassionate, safe patient care in a continuing care setting.
• Document and communicate care plans, progress, and changes to the team.
• Administer medications and IV therapies as needed per policy.
• Collaborate with care team to plan and coordinate resident care and safety.
