Position Summary:

We are seeking a dedicated, compassionate and detail-oriented Registered Nurse (RN) to join us part-time to conduct client health assessments, home safety checks, and reassessments.

Key Responsibilities:

• Conduct comprehensive health assessments for new clients
  
  
• Perform reassessments for existing clients as required
  
  
• Complete safety evaluations of client home environments
  
  
• Identify potential health risks and develop mitigation strategies
  
  
• Collaborate with caregivers, clients, and families to support safe care delivery
  
  
• Provide documentation, recommendations, and support for care plans
  
  
• Maintain accurate client health records in compliance with standards
  
  
• Support in developing personalized care recommendations
  
  
• Communicate findings clearly with the ComForCare team and stakeholders
  
  

Qualifications:

• Active Registered Nurse (RN) license in good standing with CARNA
  
  
• Minimum 2 years of nursing experience in home care, long-term care, or hospital settings
  
  
• Strong assessment and critical thinking skills
  
  
• Ability to work independently and communicate professionally
  
  
• Valid driver’s license and access to reliable transportation preferred

• Passion for senior care and person-centered service

• Excellent communication skills and attention to detail
