Fraser Health Authority is seeking a Part Time Social Worker for Hospice Palliative Community Services in White Rock, BC. You will provide social work assessments, counseling, and support to clients and families, collaborating with an interdisciplinary team to develop comprehensive care plans.

The role emphasizes managing psychosocial needs, coordinating resources, and contributing to program quality, with a strong focus on compassionate, client-centered care in a community setting.

Qualifications

• Master's in Social Work from an approved School of Social Work.
• Two years' recent clinical experience with palliative clients or equivalent.
• Current full registration with the BC College of Social Workers and valid BC Driver's license.

Responsibilities

• Provide comprehensive social work assessment and counseling to hospice palliative clients and families.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Conduct psychosocial assessments and monitor client progress.
• Coordinate referrals to community resources and support services.
• Participate in quality improvement activities and program development.
• Maintain records and complete required reports and statistics.
