Job summary

Fraser Health is seeking a Part-Time Social Worker to join the Hospice Palliative Community Services team in White Rock, BC. You will deliver psychosocial assessments, counseling, and education to clients and families, collaborating with hospitals, home care, and community agencies to support end-of-life care.

The role emphasizes interdisciplinary teamwork, evidence-based practice, and program improvement, with full participation in community-based services and local resources.

Qualifications

• Master's degree in Social Work and BC licensure eligibility.
• Two years' recent clinical experience with palliative clients or equivalent.
• Current BC College of Social Workers registration and valid BC driver's license.

Responsibilities

• Provide comprehensive social work assessment and counseling for Hospice Palliative Community Services clients.
• Collaborate with interdisciplinary team to develop and adjust care plans.
• Conduct psychosocial assessments and follow-up across care settings.
• Facilitate referrals to community resources and caregiver support services.
• Contribute to quality improvement and program development.
