Job summary

Fraser Health Authority in Surrey, BC is seeking a Relief Full Time Outreach Worker for the Overdose Outreach Team to support individuals and families affected by substance use. You will coordinate access to opioid agonist therapy, harm reduction services and community resources within a client‑centred care model.

Under RN supervision, you’ll monitor health status, assist with appointments, and advocate for benefits while promoting life‑skills development. CPR and driving are required.

Qualifications

• Substance Use Certificate plus one year of related experience or equivalent.
• CPR certificate is required.
• Demonstrated ability to work with diverse populations including those affected by substance use.
• Strong communication and advocacy skills.

Responsibilities

• Monitor clients' health and environment and report significant changes.
• Escort and accompany clients to medical appointments and errands.
• Link clients to community resources and assist with benefit access.
• Advocate for clients and coordinate with healthcare providers and agencies.
• Maintenance of outreach records and participation in care planning.
