Qualifications

• One year of recent related experience working with clients with substance use issues.
• Demonstrated ability to work with marginalized populations.
• Experience in crisis prevention and resolution.

Responsibilities

• Monitor clients' health conditions and report changes.
• Escort clients to medical appointments providing support.
• Create linkages to community resources and assist with forms.
• Administer medication and provide reminders.
• Act as a liaison between agencies and clients.
• Maintain accurate records of outreach activities.

Education

Completion of Substance Use Certificate
